Safety Walkers*

I have two safety walkers ready for each entry . Provide (2) designated Safety Walkers (SW), minimum 18 years of age, per entry space (multiple entries will require additional walkers). SW walk along each side of the entry and will monitor the entry, keeping spectators away from the entry. Entries without the appropriate number of walkers will not be allowed to proceed. All SW’s will need to report with entry at time of check-in to receive designation badges. Badges must be worn throughout the event

Designated Distributor Persons: I plan to distribute Handouts/Candy Provide sufficient DDP to safely handle and distribute your handouts/candy. DDP must be in addition to and separate from SWs, and must be a minimum age of 12 years old. No min. or max. number of DDP’s. All DDP’s must be present w/entry at check-in to receive designation badges. Badges must be worn throughout the event.

Waiver and Release of Damages against the State of Utah and the Cedar City Lions Club for participation in an approved special event on state roads.

Waiver and Release*

I do hereby release and discharge the State of Utah, the Department of Transportation, the Utah Transportation Commission, the Utah Highway Patrol, and their officers, agents, and employees, and Cedar City Lions Club (individually and as a group), from all claims, demands, and causes of action of every kind whatsoever and any damages and/or injuries which may result from my participation or the participation of my child, or group member so indicated, in the Cedar City Lions Club Independence Day Parade involving roads within the Utah State highway system or any other mater related to the parade.
